Description

Summary of Essential Functions for the Position:

 

Committed to Caring, Intake Coordinator, Licensed Vocational Nurse (LVN), The Intake Coordinator receives incoming calls from patient referral sources, gathers necessary information, and ensures quality communication between referral sources and admitting offices. Assist in answering and dispatching phone calls and messages. Works with others in a team environment. Works in conjunction with the sales team on establishing lines of communication with new referral sources and providing positive experiences to contacts.

Qualifications

Qualifications / Licensure / Certification / Knowledge / Skills / Abilities / Location / Shift / Compensation:

 

  • Licensed Vocational Nurse (LVN) preferred
  • One (1) to Two (2) years of related experience, Home Health experience preferred
  • Experience with Electronic Medical Record (EMR) System, Wellsky/Kinnser preferred
  • Strong communication skills, both oral and written with excellent interpersonal skills
  • Must demonstrate good customer relations skills and a commitment to providing quality service
  • Ability to work under pressure
  • Ability to demonstrate good organizational skills, time management
  • Proficient with a computer and Microsoft Word, Excel and Outlook software. Working knowledge of EMR database preferred
  • Acceptance and ability to demonstrate and support the core values and goals of Agency
  • Valid drivers license, automobile liability insurance and reliable transportation required
  • Travel may be required

Educational Requirements:

  • High School Diploma or GED required. Graduate of a state approved school of practical (vocational) nursing, preferred.

Working Conditions & Physical Requirements:

  • Working environment is relatively quiet and can be stressful due to deadlines, multiple tasks and general compliance of law, rules and regulations. The position requires visual acuity and dexterity, sitting, standing, some pushing, pulling and lifting up to 25 pounds.

Compensation:

  • Hourly
